If you are having technical problems please call MyChart Services at 1-855-551-6555, which is available 24 hours a day, seven days a week. If you have questions about your health information in MyChart, please call your clinic or discuss your questions with your caregiver at your next visit.
Access to MyChart is limited to authorized users who have been activated by MyChart Services. You can sign up for MyChart during your next clinic or hospital visit or you can complete and return the MyChart Sign-up Form right now. Once your MyChart Sign-up Form is processed and you have received your activation code, you are ready to sign in to MyChart.
|Step 1:||Click on the Enter your Activation Code. Enter your activation code, last 4 digits of your Social Security number and date of birth.|
|Step 2:||Choose a MyChart ID that is easy for you to remember, but hard for others to guess. Your MyChart ID must be between 5 and 14 characters and may include letters, numbers and the following symbols: period, hyphen, underscore and the at sign. Once selected, your MyChart ID cannot be changed.|
|Step 3:||Next, choose a password that is easy for you to remember, but hard for others to guess. Your MyChart password must be a minimum of 7 characters and must contain at least one number and one letter. Symbols are permitted. Please keep in mind that only you will know your password and ID; MyChart Services will not have access to this information.|
|Step 4:||Choose a security question. This will allow you to reset your password if you forget it or want to change it for security reasons.|
|Step 5:||Enter the answer to your security question.|
|Step 6:||Enter your email address. Your email address is a required field. You will receive an email notification when new information is available in your MyChart account.|
To receive access to your child’s information, simply complete and return the Child Proxy Form. You may also pick up a Child Proxy Form at your clinic.
MyChart provides access to your child’s information to help you conveniently view and manage your child’s health care.
To protect the privacy and confidentiality of health information according to state and federal laws, MyChart allows parents and guardians full chart access to their child's records until the age of 13. Once the child is 13 years old, access is limited to scheduling appointments on the child's behalf, viewing and printing immunization records and receiving health topic alerts.
